Heart of Gold LLC provides a variety of opportunities to encourage and develop relationships and natural support for our HCBS participants in their communities. We utilize the learning environment of the community to provide the services and support listed in our clients’ care plans.
Our community connector services are provided in a wide range of settings to give our clients the freedom to interact and socialize with other non-developmental disabled individuals in the community. Furthermore, these services enable them access to typical activities in the community such as visiting:
- Playgrounds
- Museums
- Local Library
- Recreational Centers
