Heart of Gold LLC offers Day Habilitation services in the form of Supported Community Connections to HCBS-DD and HCBS-SLS clients. This service provides persons with Intellectual and/or Developmental disabilities the support they need to live and participate safely in their community.
At Heart of Gold LLC, we solicit input from participants on their preferred choice of activities, thereby tailoring the program to the needs of the participant.
