Heart of Gold Inc. provides supportive residential services for individuals in a family-type setting. HOG focuses on providing services and support in a single-living and host-home setting for our HCBS participants who do not live at home with their families. We ensure the development of the functional skills necessary for them to live and participate effectively in the community while being conscious of their safety.
Our individual residential services and support/host home (DD) aims to enhance our HCBS participants’ independence through self-advocacy training and cognitive services.
Who benefits from HOG residential services?
Beneficiaries of our residential services are all individuals with developmental disabilities (our clients), the provider, and clients’ families.
Types of residential services offered by HOG
- Host Home
Heart of Gold Inc. offers Host Home services for individuals who prefer to reside in a family home with a provider. Our Host Homes are located throughout the Denver metro, Centennial, and Aurora areas of Colorado. Each host home has unique features based on its location and provider, all geared toward the client’s needs while maintaining the highest standard of care. Our case managers work in tandem with our clients, supporting them, and ensuring they are placed in the right home based on their needs for constant monitoring and follow-ups.Host Home Provider & their Responsibilities
A host home provider is one who opens their family home to support individuals who request to reside in a family home setting. They provide our clients with a safe, stable, and loving environment while making available opportunities to their access in the community and encouraging socialization.Our Host Home providers are trained in the act of caregiving, Medication Administration, QMAP, First Aid/CPR, Person-Centered Training, Customer Rights, HIPPA & Confidentiality, Abuse & Prevention, and any other training specific to the client.
- PCA (Personal assistance alternative)
PCA’s are also known as adult group homes that house a maximum of three (3) individuals. Our PCA is equipped with everything needed by the individual to be comfortable, stable, and safe. In the Host Home modality, individuals have the opportunity to reside in a family home with a trained and compassionate provider. Host Home providers open their homes to offer a safe, stable, and loving environment where our clients can thrive. These providers facilitate community access, encourage socialization, and ensure that each individual’s needs are met on a personalized level. Our case managers work closely with clients and providers to match individuals with the right home based on their specific needs and preferences.

PCA (Personal assistance alternative)
Our PCA option offers an alternative residential setting for individuals with developmental disabilities. These adult group homes accommodate a maximum of three individuals and are fully staffed with experienced and professional caregivers around the clock. Our PCAs are equipped with everything necessary to ensure our clients’ comfort, stability, and safety. Through personalized care plans and comprehensive support services, we strive to create a nurturing environment where individuals can thrive and achieve their goals.
